• Family history: negative;
• Pregnancy: uneventful;
• Birth: cesarean section; 40 weeks gestation; Wt 3650 g; Ht 50 cm; Apgar score 9;
• Postnatal development: normal growth and psycho-motor development;
• Personal history: the parents have noticed retromaleolar orange lines in the flexion creases (see pictures attached) → blood investigations → huge hypercholesterolemia (see investigations); parents have checked their cholesterol and lipid fractions and they are normal;
• Investigations:
o Hematology: HGB 10.2 g/dl (↓); RBC normal; HCT 31.1 % (↓); MCV 67 fL (↓); MCH 22.1 pg (↓); MCHC normal; WBC 9,800/ml (normal), but PN 34% and LY 60%;
o Biochemistry: done after 6 hours of fasting HDL cholesterol 46.8 mg/dl (normal); Total cholesterol 855 mg/dl (↑↑↑); Triglicerides 83 mg/dl (normal); VLDL cholesterol 17 mg/dl (normal); LDL cholesterol 791.2 mg/dl (↑↑↑); Total proteins 80,6 (normal); albumin, α1 and ß fractions normal; α2 ↑; γ ↓; GPT, GOT – normal; Urea – normal; Blood sugar 0.65 g%o;
o Abdominal ultrasound scan: normal (no fat liver!);
o ECG: normal;
o Echocardiography: heart and blood vessels – normal;
o EEG: normal
• Physical examination:
o Measurements: Ht, Wt, HC – normal;
o Examination: normal except the orange retromaleolar lines.
